Step 1: Create a new image, 200 x 100, 64M colours Select the text tool For font select "Wingdings" Select the "(" and ")" characters, these are the two phone icons (there are lots of others of course). Make them nice and large, these are 72 point. Step 2: Fill the text with your selected pattern (You could also use the text selection to cut from another image) You can use one of these patterns. Step 3: Apply the Cutout 3D sequence Add drop shadow to taste Options: There are hundreds of dingbat fonts out there, so go wild. This technique you could use with any kind of fill as well.
